Thank you for your interest in renting Concordia Lutheran High School's facility. Please fill out the group form completely including adult contact name and email. Once you have completed this form you will be a registered group user and able to check on facility availability using your log-in and password. Group must provide proof of Comprehensive - General liability, including contractual liability, insurance in the amount of $2,000,000 with Concordia Lutheran High School named as an additional insured. Such policy shall also include $5,000 medical payments coverage. Written documentation must be submitted to Concordia Lutheran High School prior to the date of use.
