Thank you for your interest in renting Concordia Lutheran High School's facility. Please fill out the group form completely including adult contact name and email. Once you have completed this form you will be a registered group user and able to check on facility availability using your log-in and password. Group must provide a Certificate of Liability Insurance – proof of Commercial General Liability insurance in the amount of $1,000,000 per each occurrence, $2,000,000 general aggregate and $5,000 medical expense per person – with Concordia Lutheran High School named as additional insured. If medical expense is excluded under General Liability, then group must provide proof of accident insurance with a minimum amount per participant of $5,000 AD&D and $15,000 medical.
